Due to a regulation review, as of July 1, 2014, the Virginia Department of Wildlife Resources will no longer be accepting Social Security documentation as proof of disability for the disabled lifetime licenses (resident and non-resident). The new process will require a physician’s affidavit form to be completed and returned with the application in order to qualify for these licenses. The application and physician affidavit form is available at:
- Application and Physician Affidavit for Virginia Residents
- Application and Physician Affidavit for Non-Residents
